It is understood the hospital informed Mr Philip’s family of the outcomes of the seven other cases it had looked into after his death. The Freeman has also written to all of the families impacted by Ms Booth in the years under review to apologise.

“We want Pauline [Mr Philip’s widow] and the family to know we have tried to learn from what happened to Ian,” a senior surgeon told the BBC. “It is clear that the conditions under which Ms Booth was working were not the optimal, and we are critically reviewing the processes in place now.”

Mr Philip’s family have called for an external review into all of Ms Booth’s cases of the last 12 years to take place. Two other families spoken to by the BBC have also called for a public inquiry.

“We want to know what went wrong, but also if this can happen elsewhere,” Liam said.

The Department of Health and Social Care, which is ultimately responsible for oversight of the NHS, said while it could not comment on individual cases it said it “expects all NHS staff to provide high-quality care”.

The Department has also said it will set up an independent panel to review deaths connected with surgery following Heart Valve replacement surgery at Freeman Hospital, Newcastle following allegations brought forward by families.

The GMC said it could not speculate on the outcome of its investigation.
