Bereaved parents ‘horrified' by Leeds maternity services report

A recent report has unveiled “significant” concerns about the safety and quality of maternity services at Leeds Teaching Hospitals (LTH) NHS Trust. The report, published by NHS England after a visit in March, pointed out issues with staffing, a challenging culture, and a failure to learn from previous incidents at the trust. To address these concerns, NHS England made 101 recommendations aimed at improving the quality of care and ensuring the wellbeing of mothers and babies. In response, LTH emphasized that urgent improvements were a top priority.

Over the past six months, the BBC has spoken with 67 families who shared their experiences of inadequate care at the trust, including instances where their babies suffered avoidable injuries or fatalities. Additionally, five whistleblowers raised safety concerns. Following the initial report by the BBC, NHS England placed the trust under the national Maternity Safety Support Programme (MSSP), which focuses on improving services in cases of serious concerns. The MSSP report highlighted several areas of concerns, including issues with safety concerns being de-escalated without resolution, a lack of essential monitoring equipment for women, and challenges with the escalation process, especially after hours.

An NHS whistleblower voiced significant concerns about the lack of progress on some of the recommendations presented in the MSSP report, citing ongoing issues that had already been identified in a previous meeting in January. A group of bereaved families described the MSSP report as “shocking and horrifying,” emphasizing the systemic failure to address concerns effectively and ensure real accountability within the trust. The report by MSSP came shortly after the downgrading of LTH’s maternity services from “good” to “inadequate” by the Care Quality Commission. Additionally, the impending retirement of the trust’s CEO, Phil Wood, sparked concerns among bereaved families regarding accountability for ongoing issues in maternity services during his tenure
