The hospital trust said it had since made significant changes to the TAVI service, including appointing a new clinical lead, increasing staffing levels, and implementing new training programs.

Christine Rymer hopes the changes mean no other family has to go through what hers did.

“To think that my mum died because of that. It’s just not right,” she says.

The hospital trust said it was deeply sorry for the pain and distress these cases had caused families, and that it was committed to learning and improving patient care.
