Nottingham maternity: Hundreds of new families added to review


The UK’s largest ever review into maternity failings is being delayed until June 2026 as it seeks to include 300 more families. Senior midwife Donna Ockenden began an inquiry in September 2022 at Nottingham University Hospitals NHS Trust, which registered more than 2,000 cases dating back to 2012. The review was originally hoped to be concluded by September, but Saturday’s announcement put the inclusion of 300 more cases to the investigation.

The review’s extension arises from an investigation into file deletions from a computer file that resulted in police launching a second investigation. Nottinghamshire Police announced that a file containing maternity documents had been deleted, leading to an investigation, and on Saturday, families involved in the review were told an additional 300 cases would join the review after a discrepancy was detected by a coroner. Nottingham University Hospitals NHS Trust said that the file had been reported as deleted in July 2024 and had since been “fully restored”.

The authorities are reviewing all the cases with the help of more than 120 clinical reviewers across the country, and the team of experts will now expand to include the new cases that have been discovered. Donna Ockenden said: “We now expect that the review will have around 2,500 families in it and publication of our final report with the extra cases will need to move.” The deadline extension has the support of Nottingham University Hospitals NHS Trust, whose Chief Executive, Anthony May, noted: “Since the start of the review in September 2022, we have worked closely with Donna Ockenden and her team, and this will continue through to the report’s publication and beyond.”

The welfare of patients who use the maternity services at Nottingham University Hospitals NHS Trust is central to the investigation, and Ms Ockenden is keen to gather as much information as possible on any failures of support or miscarriages of justice so that the trust can ensure patients receive the support they need. “While the review is ongoing, we will continue to do everything we can to improve our maternity services further,” Mr May added. The new cases discovered will add to that review and ensure the trust continues to work with the team to ensure its services meet the needs of patients in a safe and supportive way
