A family in Wales has been left distraught after attending a cremation for their loved one’s wrong body. The family, who have chosen to remain anonymous, had to have a second cremation once the error was discovered at The Grange University Hospital in Torfaen. NHS officials stated that they were “utterly heartbroken about what has happened to the family and we take responsibility for this isolated incident.”

Nicola Prygodzicz, CEO of the Aneurin Bevan University Health Board, expressed her regret and stated that words cannot express how sorry they are. The organisation believes there are “no known family of the other patient” and have met with the bereaved family to provide them with support during this difficult time. Prygodzicz wanted to reassure the public that this is an exceptional case and that they have identified the mistake through their own processes. After an initial review, the Health Board is “confident that this is down to human error in an isolated case.”

The Health Board is continuing to investigate the circumstances surrounding the incident and will update family members accordingly. Prygodzicz expressed her deep regret and stated that no words the Health Board can say or actions they can take can put this right. She promised that their full support remains with the family and that they are deeply sorry for the distress caused.
