Police discovered the bodies of Bartlomiej Kuczynski, 45, Jasmin Kuczynska, 12, and Natasha Kuczynska, 9, alongside Kanticha Sukpengpanao, 36, in a home located in Costessey near Norwich, on Friday. While Mr. Kuczynski’s passing is not being treated as suspicious, the other three victims are being investigated as murder.

Before his death, police took Mr. Kuczynski to Norfolk and Norwich University Hospital’s emergency department on December 14. The same day, he was reported to authorities as a missing person. Mr. Kuczynski left the hospital after being assessed as having “mental capacity”. Despite being voluntarily taken to the A&E due to his mental state, it is unknown whether there was a thorough mental health assessment or if he was contacted by a mental health professional.

The mental health team instructed officers to bring Mr. Kuczynski to A&E after seeking advice regarding his conduct. Mr. Kuczynski’s phone call to 999 shortly before 6 am on Tuesday caused him to express confusion and worry over his mental state, according to the IOPC. He was urged to seek medical care and contact NHS on 111, but there was no police intervention until a second phone call from a concerned member of the public at 7 am.

It remains unknown whether his family had previously raised concerns about his well-being with authorities or mental health services. The case is currently under investigation by the Independent Office for Police Conduct (IOPC), and Norfolk Police are cooperating.
