A vulnerable adult home has been fined £125,000 for failing to ensure that people not in its employ were not exposed to health and safety risks following the murder of Melissa Mathieson in 2014. Mathieson, an 18-year-old, was strangled by fellow resident Jason Conroy at Alexandra House in Bristol. The home had been warned about Conroy’s behaviour in a report by expert Hilary Grant prior to Mathieson’s death; the report highlighted that Conroy presented a severe risk to other patients and staff in the home.
The prosecution suggested that the home should have taken immediate action to protect Mathieson, but instead failed to assimilate Grant’s report. Furthermore, the report was never shared with any of the home’s employees, so Conroy’s violent tendencies were unknown. Additionally, Conroy’s top-floor room was unable to be locked.
